Why We Use a Sliding Fee Scale

But our agency counsellors/psychotherapists may not.

In England, although people have access to public / NHS counselling services, most of these services have long waiting lists, limited flexibility in terms of service delivery and the client cannot choose their counsellor.

They also usually need to go through an IAPT, or their GP which then often leads on to psychiatrists / clinical psychologists / CPN's / Care workers various / AMHT / MHT / MHHT / ASTI / A&E Mental Health Dept’s and/or, almost any other acronym of a team name you can think of.

Patients are usually coerced into a 'care' plan often created by NHS professionals, who apparently often see them as statistics and they measure evidence of movement through an uncaring system of checks and balances. but worse to come because they have been previously told, there is nobody in a position locally to give them any specialist therapy (typical example - for Trauma-PTSD or C-PTSD) required.

While some workers have access to their company’s employee assistance program (EAP), again there is limited flexibility, as well as, a set number of sessions available and you typically cannot choose the actual counsellor.

Given the above, many people turn to private counsellors / therapists at relatively low rates, or highly trained psychotherapists, at higher rates (you will get the expertise you are prepared to pay for) – people who are entirely self-employed, or work for organisation's needing to cover their expenses and who offer counseling services for a fee paid by the client/patient directly. Here the client can choose their actual counselor/therapist and while persons in private practice can generally offer more flexible services and are more readily available, there is the question of cost.

Depending on the counsellor/psychotherapist's experience, training and office expenses (Harley St. London, is expensive) private counselling services can often range anywhere from £35.00/hr to £50.00 in Milton Keynes, here in England and psychotherapy ranges from £55 - £100 and up to several hundreds of pounds per session hour, in Harley Street, just for example.

People call me George, I am the clinical lead and resident psychotraumalologist, in our local Psychotherapy Practice, called 'The Psycho-TRAUMA Surgery', in Milton Keynes. I am also a Faculty Lecturer for Convergence College in Counselling & Psychotherapy. I also work as Clinical Lead and resident Supervisor for NEWSTART Drop-In & Counselling Services. I am also available to work with clients at our Lifescript Clinic of Harley Street, in London.,W1. And have previously worked with Trauma clients for Large Multi-National Companies, such as Babcocks, Air Crash Investigation department, also London ITV station, Jeremy Kyle Show, in damage therapy, and eap agencies for the Royal Mail and HM Prison Service, to name but a few corporate organisations.

Although some semi-public services offer counselling on a fee-for-service basis and use a sliding fee schedule, one seldom sees a counsellor/psychotherapist in private practice using a sliding fee schedule.   We do here and this is the reason why.

People experience a vast range in income. Most people seeking counseling can’t wait for service for fear of the problem getting worse. People would like to choose their actual service provider typically on the basis of competence, experience and reputation. Reputable and accessible private counseling for many is not affordable regardless of how necessary it may be. I often offer pro-bono work to the deserving.

By offering a sliding fee scale we may offset the cost, of those who struggle and cannot afford to pay my average rate, by those who can afford to pay my higher rate. In this way, I can be responsive to a wider range of people given their different income levels. I do have a bottom line below which I cannot offer service and I do have an upper limit consistent with my training as a psychotherapist and years of experience and expertise. I do not negotiate my fee beyond my sliding fee scale and I will endeavour to help clients by offering them another one of our counsellors or therapists who may work at a lower rate but have been trained by Convergence College here.

My fee scale is publicly available on this page above, for every main service we provide. No guessing! Means there should be no arguments. We work under contract at all times, both verbal and written for the client / Patients to decide upon.

I have had few people at both the upper and lower points of my fee scale complain about the fee. High income earners have asked to negotiate my fee and I have turned away their business. They can afford to see me if they want to.

I have had low income earners also ask to negotiate my fee and there too, I have had to turn people away. I don’t feel badly about the former but do feel badly about the latter. Unfortunately, I too have to earn a living from my profession and any organisation who employs a psychotraumatologist (a psychotherapist specialising in Mental Health Trauma issues) will need to pay for this precious time and their extensive experience. This is our livelihood so we cannot mess around.

On rare occasions, I am asked if I will alter my fee for special circumstances. I tend not to do so any more. I cannot contemplate all the special circumstances and in fairness to me and other clients, to begin altering in one case and perhaps not another can open me/us up to a concern for unfair billing practices. As such, my fee schedule remains as noted.

Typically though, I am only 'thanked' for my sliding fee scale, by very varied status folks, across all incomes.

In the end, value is not really found in the fee. Value is found in the service and while no one is ever helpful to everyone, I am typically highly regarded for what I do. Testimonials show my work to be of the highest calibre and resulting outcomes.

I do wish more counsellors and psychotherapists would use a sliding fee scale. We enter this profession seeking to be helpful, recognising we need to earn a living. I try to achieve both as responsibly as I can. It cost me around £60,000 to train as a qualified psychotherapist / psychotraumatologist and I am a life-long learner, which also means further continuing professional development training.

The sliding fee scale also helps not only our psychotherapy practice but also our charitable organisation NewStart, to give greater service. That’s why we use it with all our services.

Need help with any traumas, single person issues, couples, family, or a neurosis of any kind, also for work, relationship, parenting or other matter? I also offer options for other services for those who cannot afford my fees.

You can read the description of each service and you can view the fee scale. Just please don’t ask us to negotiate it further. I hope that makes sense.

It would be my pleasure to be of service. DO NOT LET THEM SUFFER IN SILENCE!
